Performed final Inspection on the reinforcing steel at the following locations continuous
footings on line 1, C to G, line C, from 1 to 2, line 2, from A to C, line A, from 2 to 3 5,
bottom of mat steel In elevator pit centered at lines 3E, stairwell footings along line C 9,
from 38 to 28, line D 4, from 38 to 28, line 2 8, from C 9 to D 4, column footings at
lines F1, E1, D1, C1, A 1, E2, D2, C2, A2, C3, C4, C5 and A3 Performed preliminary
Inspection on the reinforCing steel for footings around elevator pit along lines 2 3, from
E 1 to E 5, line E 5, from 2 3 to 3 6 and line 3 6, from D to E 5 ReinforCing steel was
checked for grade, Size, lap, placement and clearance ReinforCing steel was secured
with tie-Wire and supported above grade Observed the placement of 55 yards of MBI
4000 pSI (MIX 24F4NE5200) concrete In bottom of elevator pit Concrete was placed by
1 pump and was mechanrcally consolidated Cast one (1) set of four (4) 6" X 12"
concrete test cyllnpers Slump 41/4", concrete temperature 61F, air temperature 54F
Observed the placement of 500 yards of MBI 3500 pSI concrete (MIX 35-1 KV) In
footings listed above except around elevator pit Concrete was placed by pump and
was mechanrcally consolidate Cast one (1) set of four (4) 6" X 12" concrete test
cylinders from load 2 Curt (Melli Construction) was notified of on-site test results
slump 41/2", concrete temperature 59F, air temperature 56F
Items requiring correction: In elevator pit footing on line 3 6, from D 5 to E 5, rebar In
area of step IS missing bent bars and one (1) set of diagonal bars as shown on 7/S5 1
To the best of our knowledge, the work Inspected was In accordance With plans
and applicable workmanship provIsions of the I B C , except If noted above Contractor
was asked to notify our office when additional services are needed
