. Performed visual inspection of reinforcing steel In the following location cut-out slab area for new restrooms.
Reinforcing steel was checked for grade, Size, lap, placement and clearances where applicable Reinforcing steel was
secured uSing tie wire and supported on concrete dobre blocks Reinforcing steel was placed In accordance wIth
approved design drawl'lgs and specifications Performed visual Inspection dunng placement of concrete In the locations
listed above Concrete was placed by truck and hand screeded. Cast one set offour (6x12) concrete test cylinders To
the best of our knowledge, the work Inspected above was In accordance With the bUilding department approved design
drawings, specifications and applicable workmanship provIsions of the International BUilding Code The Contractor was
asked to notify our office when additional services are needed

Performed continuous inspection dUring the installation of #4 epoxied rebar dowels In the following locations (50) #4
rebar dowels at 1 '-4" on center Rebar dowels are located In eXisting floor slab, In the far South East room of the
construction site Rebar dowels Installed as per engineers specifications to detail 2/S2 Prior to installation the predrllled
hole were cleaned uSing a nylon brush and compressed air Verified minimum embedment depth of 4 Inches The
above listed epoxy anchors were Installed uSing Simpson Set 22 epoxy with ICC# 5279 The epoxy anchors were
Installed In accordance with approved Job-site plans, the manufacturer's directions and the latest reVISions dated July 17.
2006, In addition tere was the presence of an oily residue on the rebar dowels, the contractor however cleaned most of
the 011 off of the qowels before installing them To the best of our knowledge, the work Inspected above was In
accordance With the building department approved deSign draWings, specifications, and applicable workmanship
provIsions of the International BUilding Code Contractor was asked to notify our office when additional services are
needed
