ConsuHing · Testing DAILY FIELD REPORT Date Performed: September 20, 2006 Report Number: 722-60303-10 Client: Springfield School DIstrict #19 Project Name: A3 Academy of Arts & 1890 N 42nd Street Academics Springfield, OR 97477 Project Location: 611/615 Main Street SpringfIeld, Oregon Contractor: Melli Construction Co Building Permit No.: COM 2006-00836 Type of Inspection: Epoxy Weather: InSide Field Observations and Comments Performed continuous Inspection durrng the installation of 5/8" all thread epoxied anchor bolts In the following locations (6) at grrdllne 2 In between grrdllnes A and B, (4) on grrdllne 2 between grrdllnes Band C (15) on line 2 on the other side of the wall between grid lines A and D All anchors are for Simpson Hold Downs, HD2A, between rafters and wall Prror to Installation the predrrlled holes were cleaned uSing a nylon brush and compressed air Verrfled minimum embedment depth of 2 Inches The above listed epoxy were Installed uSing Hlltl Hit Hy 20 epoxy with ICBO # 4815 Contractor stated the nuts would be tightened after epoxy has cured The epoxy were Installed In accordance with manufacturer's direction and englneerrng approval needed In addition all anchors Installed on this date need englneerrng approval for "as bUilt" condition. There were only two Inches of embedment through the open cells of the block Plans called for a minimum of 4 inches PSI has received an RFI stating 2 Inches IS acceptable from the engineer. Performed continuous Inspection dunng the Installation of 5/8" all thread epoxied anchor bolts In the following locations, (12) along gndllne 1 as per detail 11/S2, (3) on gndllne 1 as per detail 12/S2, (3) along gndlrne between D and C 5 as per detail 11/S2, (6) on gndlrne 3 between A and 8.5 as per detail 7R/S2, (5) anchors on gnd line 2 between B 5 and C 5 as per detail 6R/S2. These were Installed uSing Hlltl Hit Hy 20 with ICBO# 4815 with a minimum embedment depth of 6 Inches as per engineer's specificatIons Pnor to installatIon the prednlled holes were cleaned uSing a nylon brush and compressed air, Venfled minimum embedment depth of 4 Inches for dowels Inserted honzontally and venfied embedment depth of 6 Inches for dowels placed at an angle the above listed epoxy were Installed uSing Simpson Set 22 epoxy With ICC # 5279 Contractor stated the nuts would be tightened after epoxy has cured The epoxy were Installed In accordance With approved Job-Site plans and the latest reVISions dated September 14, 2006.
