Field Observations and Comments
Performed continuous inspection dUring the installation of 5/8" all thread epoxied anchor bolts and #4 rebar dowels In'
the follOWing locations 5 anchors on gndhne 2, between grldllnes A and 8, with 2 Inch minimum embedment depth
uSing Epcon System screen tubes as per engineer's speCifications, 5 anchors on gndhne 3, between A and B with a
minimum of 4 Inch embedment as per engineer's speCification Also (5) #4 rebar dowels with a minimum of 4 Inch
embedment at the southeast outSide corner of the construction site Prior to installation the prednlled holes were
cleanedc using a nylon brush and compressed air Venfied minimum embedment depth of 2 & 4 Inches The above
listed epoxy were Installed uSing Hiltl Hit Hy 20 epoxy with ICBO #4815. Contractor stated the nuts would be tightened
after epoxy has cured The epoxy were Installed In accordance with manufacturer's direction and RFI on file To the
best of our knowledge, the work above was In accordance with the bUilding department approved design drawings,
speCifications and applicable workmanship proVISions of the International BUilding Code. Contractor was asked to notify
services are needed
