Performed final inspection on the reinforCing steel for slab on grade on line A to D and
1 to 8 Reinforcing steel was checked for grade, Size, lap, placement and clearance
ReinforCing steel was secured with tie-wire and supported above grade with doble
blocks
Observed the placement of 227 0 cu /yds of Eugene Sand and Gravel's 3000 pSI
concrete (MIX 2144) In areas listed above Concrete was placed by pump and was
consolidated by Vibratory laser screed Cast two (2) sets of four (4) 6" x 12" concrete
test cylinders Dave Krull (Chambers Construction) was notified of on-site test results

Slump - 4 3/4"
Concrete Temperature - 72F
Air Temperature - 46F
Set 2
Slump - 31/2"
Concrete Temperature - 70F
Air Temperature - 47F
Mr Krull was asked to notify our office when additional services are needed
To the best of our knowledge, the work Inspected was In accordance with plans dated
01-22-07 and applicable workmanship provIsions of the I B C , except If noted above
