FIELD INSPECTION REPORT

PROJECT. Bayberry Commons - Springfield
ADDRESS: 2211 Laura Street - Springfield, OR

08/16/06: As requested CTI representative was on site this date for epoxy anchor
Installation for seismic holddowns (PHD2 & PHD5). Contractor installed 5/8" all thread with 6"
embedment into slab on grade. Contractor used Hllti Hy-150 Max batch #003219L4 expiration
2/2007, nylon brush and forced air used to clean holes, Contractor to re-drill (4) holes that did
not meet embedment reqUIrement, (2) In east wing & (2) in south wing tracked on NCL Item #3.
Note, holes at entryway for PHD15 appear to be diamond bit cored and the anchors are not yet
installed.
