Contractor: Melli Construction BP No.' COM2005-01772 Periodic Inspection: 0'
Performed inspection on reinforCing steel on 8" CMU sound wall along south property
line (footing to 4'0") ReinforCing steel was checked for grade, Size, lap, placement and
clearance, where applicable Cells were relatively clean and free of excessive mortar
projections Observed the placement of 7 0 cu /yds of MBI 7 sack grout In wall noted
above Grout was placed by pump and was mechanically consolidated Wall was
grouted solid Grout slump - :t9", grout temperature - 78F, air temperature - 80F
Note contractor stated he spoke to EOR regarding compression testing of prisms He
stated none were needed for thiS Job
To the best of our knowledge, the work Inspected was In accordance with plans and
applicable workmanship provIsions of the I B C , except If noted above Contractor was
asked to notify our office when additional services are needed
