Inspection of: Reinforced Concrete Weather. Clear Contmuous Inspection: 0
Contractor: Melli Construction BP No.. COM2005-01772 Periodic Inspection: 0
Performed inspection on the reinforcing steel for loading dock slab Reinforcing steel
was checked for grade, Size, lap, placement and clearance Reinforcing steel was
secured with tie-Wire and supported above grade Observed the placement of the first
truck of MBI 3500 pSI concrete (MIx 35) In areas listed above Concrete was placed
directly from the truck and was mechanically consolidated Cast one (1) set of four (4)
6" x 12" concrete test cylinders JR (Melli Construction) was notified of on-site test
results slump 3 3/4", concrete temperature 74F, air temperature 64F, air entrainment
40%
To the best of our knowledge, the work Inspected was In accordance With plans dated
12/19/05 and applicable workmanship proVIsions of the I B C , except If noted above
Contractor was asked to notify our office when additional services are needed
