Performed Inspection on the reinforCing steel for four (4) light pole bases In south
parking lot area ReinforCing steel was checked for grade, Size, lap, placement and
clearance ReinforCing steel was secured with tie-Wire Anchor bolts were checked for
size and quantity and were secured In-place with templates
To the best of our knowledge, the work Inspected was In accordance with plans dated
12/19/06 and applicable workmanship prOVISions of the I B C , except If noted above
Contractor was asked to notify our office when additional services are needed
