Performed continuous inspection dunng the Installation of three (3) #5 x 35"
replacement vertical dowels In floor slab as follows one (1) at wall end near lines F4
and two (2) In pier at E6 3 Pnor to installation, the prednlled holes were cleaned With a
brush and compressed air Venfled minimum embedment depth of 5" Dowels were
secured With Simpson Set 22 epoxy (ICC ES ER5279, expiration date 11/07)
To the best of our knowledge, the work Inspected was In accordance With EOR verbal
Instructions and applicable workmanship proVIsions of the I B C , except If noted above
JR (Melli Construction) was asked to notify our office when additional services are
needed
