Performed inspection on the reinforcing steel In the 6" and 8" CMU wainscot wall on the
north Side of the bUilding (slab to top) Reinforcing steel was checked for grade, Size,
lap, quantity, spacing and clearance, where applicable Observed the placement of
55 cu /yds of MBI 75 sack grout In walls noted above Grout was placed by
pump and was manually consolidated Grout slump 8", grout temperature 76F, air
temperature 78F Walls were grouted solid except cells at wall end near lines F4 and
In pier at E6 3 Vertical #5 bars need to be epoxied Into slab (per EOR) before grout IS
placed at these two locations JR (Melli-Construction) was adVised
To the best of our knowledge, the work Inspected was In accordance with plans dated
12/09/06 and applicable workmanship provIsions of the I B C , except If noted above
JR was asked to notify our office when additional services are needed
