Inspection of: Reinforced Concrete Weather Clear Continuous Inspection: I::;
Contractor: Melli Construction BP No COM 2005-01772 Periodic Inspection 0
Performed Inspection on the reinforcing steel for center third of stemwall on west Side
and east half of footing along south Side ReinfOrcing steel was checked for grade,
Size, lap, placement and clearance Reinforcing steel was secured with tie-wire and
supported above grade Observed the placement of the first truck of MBI 3500 pSI
concrete (MIX 35) Into west stem wall Concrete was placed by pump and was
mechanically consolidated Cast one (1) set of four (4) 6" x 12" concrete test cylinders
Contractor was notified of on-site test results slump 2", concrete temperature 78F, air
temperature 75F '
To the best of our knowledge, the work Inspected was In accordance with plans dated
12/19/05 and applicable workmanship prOVISions of the I B C , except If noted above
Contractor was asked to notify our office when additional services are needed
