Performed visual inspection of reinforcing steel In the following locations grldllne 9-17/A top out Reinforcing steel was
inspected for Size, grade, lap, placement and clearances where applicable, Reinforcing steel was placed In accordance
with approved design drawings and specifications and detail 07/04. All cells were relatively clean and mortar projections
are withm the limits of the International BUlldmg Code. Performed continuous Inspection dunng placement of grout m the
locations listed above. Grout was placed by pump and mechanically consolidated, Only cells contalnmg relnforcmg
steel were grouted solid Empty cells were filled with wire mesh, To the best of our knowledge, the work Inspected
above was In accordance with the building department approved design drawings, specifications and applicable
workmanship prOVISions of the International BUlldmg Code Contractor was asked to notify our office when additional
services are needed.

Met on-site with Horning Construction representative to perform special Inspection on A~25 high strength bolts In the
primary structural roof connections Skidmore-Wilhelm test was performed and bolt crew was Instructed on "turn of the
nut" method of bolt tensioning Two (2) rafter/column lines were continuously Inspected dUring "snug tight" operation to
ensure compliance with AISC manual instructions by Horning Construction Additional Inspection Will be required upon
completion of "snug tight" tensioning (prior to final 1/3 turn) and again after final 1/3 turn has been performed To the
best of our knowledge, the work Inspected above was In accordance with the bUilding department approved deSign
draWings, specifications and applicable workmanship provIsions of the International BUilding Code The contractor was
asked to notify our office when additional services are needed
