Dear Mr. Ferguson:
At your request, your property commonly known as 702 and 704 South 51st Place, Springfield, Oregon,
also known as Lane County Reference Number 18020422, Tax Lot Number 01900 has been changed. The
unit currently addressed as 702 South 51st Place will be changed to 5091 Forsythia Drive. The second unit
known as 704 South 51 st Place will remain unchanged.
As we discussed, addresses are normally assigned by the streetJrom which the property takes vehicular
access 3nd or has frontage. Afier reviewing your request, it was determined that with your unit using
Forsythia Drive as the main and front access to your residence, the needs of emergency services would b~
beller suited changing your address from 51 st Place to Forsythia Drive.
