Dear Mr. and Ms. Allen:
I am writing in regards to a heat pump 'and air handler permit that was issued for your
property at the above location on February 7, 2008. Although a mechanical permit was
obtained for the installation, an electrical permit was not obtained. We have contacted
Associated Heating and Air Conditioning inquiring about the electrical work but have not
received an application for the electrical permit. To date, we still do not having a valid
electrical permit for this project. Because we have made several attempts to have the
contractor obtain the required electrical permit for this project, and, with no response
from the contractor, the fees for the electrical permit will be doubled. The fee due is
$125.84. The breakdown of this fee is as follows:

Please contact your electrical contractor and request they obtain this permit as soon as
possible. Once the permit has been issued, an inspection is required to be performed to
ensure electrical safety on this installation.
