PMB #176 Springfield, OR 97476 RE: 743 Edoemonl Wav' Sorinofield - Tree Damage Report WO.#: 16745.MJD As requested, a site visit was performed on April 5, 2005 in order to observe damage done at the above address due to a tree falling. The tree fell on the upper portions of the east side gable end. The Interior living area of the structure was not observed due to lack of access. However, Bob Bash of Pacific Fire & Flood reported that no cracking or other signs of damage were observed in the interior living area. ' Observations in the attic in the area of the tree found two damaged trusses, The gable end truss has been damaged in a way that compromises the integrity of the connection at the peak, The truss located directly west of the gable end truss has been similarly damaged, However, this truss has been damaged more severely. Additionally, the gable end truss member supporting the 3' overhang on the northeast comer of the structure has been damage to the point of needing replacing, Repair to the structure shall be performed as described below: Gabla End Truss: A piece of W.thick'plywood measuring 24" wide by 1S' from peak of truss down shall be applied to the in side of the truss, The plywood shall be nailed .wlth Sd nails at 3' on centers to each member of the truss leading to the peak, . Truss Oirec:tly t~ West of Gable End: A piece of~' thick plYWOOd measuring 24' wide by 16' from peak of truss down shall be applied to both sides of the truss, The plywood shall be nailed with ad nails at 3' on centers to each member of the truss leading to the peak, Overhang Member: This 2x4 member that overhangs 3' shail be removed for the length of the overtiang plus S' back into the structure, This member may then be replaced with anew 2x4 to match pre-existing conditions.
