Appl i cation is hereby made by the unders i gned, presently o\~ni ng property outside

It is understood that before connection is made, the following charges must be
paid:
1.) $4,50 per front foot of the property for the first 150 feet in depth
to be served by such City 5ewer line in lieu of an assessment against
the described property, An additional $0.03 per square foot will be
charged for any additional property beyond the first 150 feet,

In addition to the above, sewer user charges as well as the cost of house con-
nection must be assumed by the owner,
Itis herebY agreed that the above charges shall represent the total charges
to be paid for connection and service while the above described property is pending
annexation. Should for any reason the annexation request be denied, sanitary sewer
service must be physically terminated by the owner within 60 days of such date of
denial or the City will terminate service at the expense of the owner.
