1 Demolition: After demolition is complete, sewer is capped or septic is pumped and filled and inspection is
requested and approved, and all debris is removed from the site.
2 Septic Tank Pumped: After septic tank has heen pumped and filled. Please provide the inspector with receipt and
verification from company performing pump and fill.

information hereon is true and correct, and I further certify that any and all work performed shall be done in accordance with
the Ordinances of the City of Springfield and the Laws of the State of Oregon pertaining to the work described herein, and
that NO OCCUPANCY will be made of any structure without permission of the Community Services Division, Building Safety.
I further certify that only contractors and employees who are in compliance with ORS 701.005 will be used on this project.

DEMOLITION PERMIT APPLICATION~
Your demolition permit is currently being .processed. There may be a slight
delay, of up to' 2 working days for small structures, due to the time required
to review the history of the structure to determine if it needs to be documented
before demolition. This documentation is for archival purposes only and will
not affect the granting of thedemol ition permit. If the structure is very large
or complicated the documentation' process may take up to a maximum of 4 working
days . Documentation will consist of photographing thebuildi ng , taking
measurements and making scaled drawings. The documentation will be undertaken
by the City at no cost to you. Documentation is being done on all structures
dated prior to' 1940 that may have historic importance to the City's development.-

An age cut-off of 1940 was chosen because this is the date that the National
Parks Service and the Springfield Development Code use to determine potential
historical' significance.

The applicant is hereby notified that any redevelopment of the subject site must comply
with all of the applicable laws, codes, ordinances, policies and plans in effect at the time .
the redevelopment proposal is accepted as complete for City review. This would inClude
, correction of substandard conditions associated with the'present development. Examples

Re: Pre-demolition asbestos abatement of storage buildings at:
3483 Game Farm Road Springfield, Oregon
In July of 2003, an inspection and site survey for asbestos containing building
materials were conducted at the above-mentioned address. From the
homogeneous list of building materials established, 1 type of asbestos was
present in the materials sampled. These materials are required by law be
properly abated prior to the demolition of this structure.
On May 10, 2004 the asbestos containing building materials found in this
investigation were properly abated by the Pacific Environmental Group Inc. of
Eugene Oregon, The waste generated is scheduled to be disposed of at the
Lane County Solid Waste Disposal Facilities Asbestos Disposal Site.
The house and adjacent storage buildings, following this exercise, are
considered ready for demolition pursuant to the rules specified in DEQ Division
340-248-0270 and LRAPA's Title 43-015-10-A&B regarding asbestos,
All documentation from these activities and the credentials of those who
performed them are on file at the offices of Pacific Environmental Group Inc,

Re: Pre-demolition asbestos survey, Main house, Large and small out buildings
directly Northeast of the Main house.
3483 Game Farm Road, Springfield, Oregon.
On July 7,2003 an inspection and site survey for asbestos containing building
materials was conducted at the above-mentioned address. From the
homogeneous list of building materials established, 8 types of Roofing, window
glazing compound, ceiling tiles and textures, drywall composites, plaster wall
composites and 3 types of flooring materials were sampled and analyzed by an
independent laboratory, Magnum Environmental Services.
The results indicated that asbestos was present window glazing compound
materials sampled, in the windows on the two small out buildings directly
northeast of the main house,
On May 10, 2004 the asbestos containing building materials found in this
investigation were properly abated by the Pacific Environmental Group Inc, of
Eugene Oregon. The waste generated is scheduled to be disposed of at the
