ITEM TITLE: CITY OF SPRINGFIELD WELLNESS CLINIC CONTRACT
ACTION REQUESTED:
Authorize City Manager to sign a 3rd Amendment to the agreement with Willamette Community Health Solutions DBA Cascade Health Solutions to extend the contract in the amount not to exceed $200,000 to provide primary and preventative medical
services for the City’s Wellness Clinic.
ISSUE STATEMENT:
Human Resources is requesting a one month extension for the contract currently in
place with Cascade Health Solutions to provide medical staffing and services for
our Wellness Clinic while a successor contract is finalized.

DISCUSSION/ FINANCIAL IMPACT:
Primary and preventative care medical services can be provided by a Family Nurse Practitioner (FNP) for our employees, retirees and their dependents enrolled on our
medical health plan. The current $200,000 contract represents the cost to provide
these services at our Wellness Clinic.
The cost to extend the current contract from February 28, 2017 through March 31, 2017 is $16,700.
Staff recommends that Council authorize the City Manager to sign the agreement
with Willamette Community Health Solutions DBA Cascade Health Solutions to extend the current contract until March 31, 2017 to provide primary and
preventative medical services for the City’s Wellness Clinic. This extension will allow staff some time to negotiate a successor agreement which will coincide with the opening of a new clinic located in City Hall.
