ISSUE
STATEMENT:
At the October 22, 2007 Work Session, Council heard a summary of the
need for an ambulance rate increase and Council support was requested.
Council agreed to hold a public hearing on the request during their next
regular meeting on November 5,2007.

FINANCIAL
IMPACT:
In 2002, the federal Centers for Medicare and Medicaid Services (CMS)
initiated a series of ambulance reimbursement reductions that have resulted
in a loss of income on all Medicare and Medicaid patients transported by
ambulance. A May 2007 report of the federal General Accountability Office
(GAO) confirmed that the new reimbursement level is an average of 6%
below the actual cost of providing services. The impact of the reduction on
ambulance revenues is being felt locally in Eugene and Springfield,
statewide, and around the nation. The financial impact in Springfield has
resulted in depletion of the ambulance fund reserves and recommendation
for an interim rate increase.
An ambulance rate increase from $900 base rate to $1,150 and mileage
increase from $15.00 to $15.50 will raise approximately $290,000 annually.
The interim rate increase will provide limited relief to allow further evaluation
while a long term strategy is being developed.

ISSUE:
A signicificant reduction in Medicare/Medicaid ambulance reimbursements and lack of adequate funding
for fire facilities and fire/rescue vehicles is challenging the budget ofthe Fire & Life Safety Department. A
Council Work Session on this topic is scheduled for October 22. The purpose of this document is to
summarize the challenges and provide supplemental information for Council decisions.
BACKGROUND:
MedicareIMedicaid - A phased reduction in ambulance reimbursements since 2002 has resulted in a loss
of over $181 on every Medicare patient transported. Since Medicare patients comprise over 67% of all
transports, at this rate, unless corrective action is taken, the ambulance fund reserve will be depleted and a
deficit of $96,000 will occur by the end ofFY 08. As a short term solution, an ambulance rate increase of
$250 Base Rate and 50 cents per mile is recommended for public hearing and implementation on
November 5. This would raise approximately $290,000 annually and allow time to consider various
options for long term resolution. A long term plan will be ready for Council consideration in time for
potential implementation at the beginning ofFY 09.
Virtually all urban jurisdictions in the nation are experiencing the same problem with Medicare. Eugene
Fire & EMS is considering a similar ambulance rate increase and will address this issue at a November 28
Council m~eting. Both cities have discussed joint efforts to lobby for Medicare reform legislation through
their Intergovernmental Relations offices. A current proposed piece of legislation regarding an emergency
5% increase in Medicare reimbursements if passed, would raise approximately $71,000 annually. While
this would help, far more sweeping reform is needed to fill the gap created by reductions in
reimbursements.
Fire Facilities - No funds have been set aside for rehabilitation of existing fire facilities or construction of
new or replacement facilities to accommodate groWth in the city. Information included with the FY 08
budget estimated the five year impact of facility needs at $17 million. Of this, Council has approved $2.6
million for construction of a new Thurston Fire Station. As part of the FY 09 budget process, a description
of the projected time lines and cost for additional facilites and options for funding will be included. .
Fire Vehicles - Funds provided for fire vehicle replacement (Fund 713) have been less than 10% of that
needed to replace used vehicles within their lifecycle. As a result, one of the last two fire engines was
leased rather than purchased. Information included with the FY 08 budget estimated the one time catch up
replacement funds needed at $1.75 million and $500,000 thereafter. As part of the FY 09 budget process, a
description of the projected timelines and cost for vehicles and options for funding will be included.
RECOMMENDED ACTION:
Hold a public hearing and approve the interim ambulance rate increase request. Action on fire facilites and
vehicles will be considered as part of the FY 09 budget.

AIO CALLS
The Aid Call rate may be used whenever patient treatment services are rendered,
especially when an advanced life support intervention was administered, but the patient
was not transported.
This charge may also be used for expired patients under the following conditions:
1... Calls where death is not certain and assessment and/or care are necessary
to determine death will be handled as an "aid call." Cases of obvious death,
i.e. rigor mortis, decapitation, etc., where NO evaluation or treatment is
performed, will be handled as "no patients".
2... Use of the monitor/defibrillator, solely for the purpose of confirming death,
rather than for active patient care, will not constitute a charge, however, if the
monitor/defibrillator is used for resuscitation, appropriate charges will be
made.

MULTIPLE PATIENT SITUATIONS
When a medic unit transports more than one patient from the same incident or from
unrelated incidents, each patient is charged a full base rate. This is in recognition that
each patient receives the same care and transportation services as when only one
patient is transported. However, an adjustment will be made for mileage charges. In
instances where more than one patient is transported in the same medic unit, the
normal mileage charge will be split among the number of patients transported, i.e. if two
patients are transported, they will each be charged a full base rate but will only be
charged 50% of the normal mileage charge, three patients would be charge 33 1/3% of
the normal mileage charge.

A RESOLUTION OF THE COMMON COUNCIL OF THE CITY OF SPRINGFIELD
AMENDING THE MASTER SCHEDULE OF MISCELLANEOUS FEES AND CHARGES,
RATES, PERMITS AND LICENSES IN ORDER TO AMEND THE AMOUNT OF THE
AMBULANCE AND EMERGENCY MEDICAL SERVICES USER FEES.
WHEREAS, the City of Springfield is authorized to establish user
fees in exchange for a particular service which benefits the
party paying the fee in a manner not shared by others paying the
fee; and,
WHEREAS, the fees established herein are to compensate the City
for providing the particular services set forth in the Master
Schedule; and,
WHEREAS, the City has determined that the fees for the Ambulance
and Emergency Medical Services User Fees should be amended to
compensate the City for providing the Ambulance and Emergency
Medical Services set forth; and,
WHEREAS, except for the increase in the Ambulance and Emergency
Medical Services User Fees as specified herein, the fees sei
forth in the Master Schedule remain unchanged and shall continue
in full force and effect; and,
WHEREAS, the opportunity for public comment at a public hearing
has been provided prior to the adoption of this Resolution.
NOW THEREFORE, be it resolved by the Common Council of the City
of Springfield as follows:
1. To amend the Ambulance and Emergency Medical Services User
Fees as set forth in Exhibit A, the "Master Schedule of
Miscellaneous Fees and Charges, Rates, Permits, and
Licenses" attached hereto and incorporated herein by
reference.
2. Except as specifically amended by this Resolution the
"Master Schedule of Miscellaneous Fees and Charges, Rates,
Permits, and Licenses" shall remain unchanged and in full
force and effect.
3. Future Resolutions of the Common Council may be adopted to
amend, add or delete any rate, permit fee, license fee, or
other fee or charge as may be necessary to compensate the
City for providing the particular services set forth in the
Master Schedule.

4. In the event any particular rate, permit fee, license fee,
or.other fee or charge set forth in the Master Schedule
shall be held invalid by operation of law or any court of
competent jurisdiction or the enforcement of any fBe
restrained by such court pending the final determination as
to its validity, the remainder of the rates, permit fees,
license fees, fees and charges specified in the Master
Schedule, Exhibit A, shall not be affected thereby and
shall remain in full force and effect.
5. This Resolution shall take effect on November 6, 2007

Graduated Fee Schedule based on fine amount imposed or Council Resolution
