IN THE AMOUNT OF $45,105.25, INCREASING THE TOTAL APPROVED CONTRACT TO $128,367.75, AND CHANGING THE CONTRACT COMPLETION DATE TO JUNE 30, 2015. ISSUE STATEMENT: The majority of the work on the City Council directed Slurry Seal project has been completed; however, a City Council approved change order is required due to the condition of the streets included in the project. ATTACHMENTS: 1. Change Order #1 DISCUSSION/ FINANCIAL IMPACT: At the direction of the City Council during the June 9, 2014 Work Session DPW staff evaluated the best way to expedite a slurry seal solicitation under state and local procurement rules in order to accomplish a project before the end of summer. Settling on the formal quote process, a solicitation was posted and quotes were received with a contract awarded to the lowest responsible bidder prior to August 1st. The contract required the contractor to complete the work on or before September 30, 2014. The contractor completed the work required on September 30, 2014, however due to weather conditions a portion of the slurry did not cure. Payment was withheld for the work in question and the contractor has agreed to return in late spring 2015 to reapply slurry to the segments in question. To facilitate this through the contract, change order #1 extends the contract to June 30, 2015. Also included in this change order is a request to increase the Slurry Seal Crack Seal quantity. This change is necessary as the original solicitation was based on a best guess of need; however, the condition of the streets chosen for the project required the use of more product to adequately prepare them for the slurry seal. There is sufficient funding in the project account and Street fund reserve to cover the total cost of change order #1. Staff recommends that the City Council approve change order #1. A4 Rev 03-13-13 CITY OF SPRINGFIELD, OREGON Change Order Number 1 Project No: P21105 Project Title: FY 2015 Slurry Seal It has been found to be in the best interest of the City to make certain changes regarding certain items of work under this contract as indicated below. Said changes do not in any way affect the general requirements of the contract, nor the Specifications for other work items thereof. This is a work agreement and hereby becomes a part of, and shall be governed by, the requirements of contractual documents. The Slurry Seal Crack Sealing needs to be increased in the contract quantities as the condition of the streets selected for slurry seal application were in worse condition than anticipated when quotes were solicited. The increase in quantity increases the total final contract price more than 30% of the original triggering the need for a change order. The Slurry Seal Crack Seal quantity has been increased at the original unit price, as the original unit price is in line with other bids and quantities solicited in our region. The completion date is being changed from September 30, 2014 to June 30, 2015. This change is to allow for the contractor to return in the spring to complete necessary repairs.
