Ordinances of the City of Springfield and the Laws of the State or Oregon pertaining to the work described herein, and that NO
OCCUPANCY will be made of any structure without permission of the Community Services Division, Building Safety. I further
certify that only contractors and employees who are in compliance with ORS 701.005 will be used on this project. I further agree
to ensure that all required inspections are requested at the proper time, that each address is readable from the street, that the
permit card is located at the front of the property, and the approved set of plans will remain on the site at all times during
construction.

----=_ We propose to furnish all materials and perform all labor necessary to complete the following:
(OPT
ION A: 1:Install -1650mi1 PVC_Mechanically fastened membrane roofing system - White,
over one (1) layer fire rated base sheet, over existing roof.
_ _ 2. New system will encapsulate parapet walls, terminating onto new outer edge metal
flashings. grease fans.
3. Install chemical resistant membrane around two (2) g
4. Install protectiontwalk pads to and around HVAC units.
5. Install new pressure treated support blocks.
6. Install new base flashings for pipes, vents and rooftop units.
7. Remove two (2) vents on south side.
8. 15 year warranty on IB material.
9. Install tapered insulation around old angle iron supports on top of parapet wall.
10. Provide permits.
11. Remove our roofing debris from job site.
BID PRICE: $24,100.00
OPTION B: 1. Same as Option A except install TPO 60mll mechanically fastened membrane roofing.
2. 20 year warranty on TPO material.
BID PRICE: $22,900.00
Notes: 1) Electrical conduit on top of parapet walls relocated by owner's representative
or by River Roofing on a time and materials basis.
2) Remove and reinstall satellite as required, realignment, if needed, to be done by others.
3) To remove old roofing and install new membrane roofing system over 1/4" fire rated
DensDeck, add $9,500.00 to either option.
Price good for 30 days. Workmanship is guaranteed for five (5) years. Manufacturers roofing warranty is as stated above.
During demolition debris may sift Into attic areas. Please take measure to cover valuables. For Visa/MC, add 3% surcharge.
Payment to be made as follows:
El Total due upon completion LXDeposit of 10% with balance due upon substantial completion
In the event it becomes necessary to employ an attorney or institute a lawsuit to collect any payment due River Roofing under this Agreement
or any modification to this Agreement, River Roofing shall be entitled to recover its attorney fees, costs and disbursements incurred. A late
payment charge of 2% per month will be added to any account not paid in full within 10 days after billing. A rebilling chargd of $25 will be
added to all past due accounts. An administrative fee of 25°5 will be assessed on all deposits if job is cancelled. Visa/MC for deposits only.
The proposal set forth above may be withdrawn by River Roofing if not accepted within 30 days. The terms and conditions on the reverse
are part of this contract.
