OCCUPANCY will be made of any structure without permission of the Community Services Division, Building Safety. I further
certify that only contractors and employees who are in compliance with ORS 701.005 will be used on this project. I further agree
to ensure that all required inspections are requested at the proper time,that each address is readable from the street,that the •

2.) Install 60 mil TPO membrane roof..
3.) Install flashings on penetrations.
4.) Install TPO clad drip edge and gravel stop.
5.) Clean ground and gutter of debris from roofing project.

NOTE:During roof removal, debris may sift into attic. Please cover any valuables.
McKenzie Roofing Inc. guarantees their workmanship on the project for five years
upon completion. All material is guaranteed to be as specified, and the above work
to be performed in accordance with the drawings and specifications submitted for
above work and completed in a substantial workman like manner for the sum of(SEE
ABOVE) with payment due in full upon completion of project
Any alteration or deviation from the above specifications involving extra cost, will be
executed only upon written orders, and will become an extra charge over and above the
estimate.
Al agreements contingent upon accidents, delays,or material price increases beyond our control. Owner
to obtain permits,carry fire,tornado and other necessary insurance upon above work.
