As requested by Jim Culver of E. A. White Construction CO., Earth Engineers, Inc. (EEI) representative Greg Thibeaux,
E.I.T. was on site to perform special inspection of two (2) Simpson Strong-Tie ATS anchor tie-down system units located
at grid line 9 at M and approximately 9 at J (refer to drawing: 2nd Floor Framing Plan, 1-S2.2, Sheet OL, Job No:-
130915A, Candlewood Suites, dated 11/11/2013 provided to us by Jim Culver of E. A. White Construction Company).
The units were replaced after the initial inspection (on April 11, 2014) because they had been installed incorrectly. Upon
arrival on site, EEI rep observed the contractor, Jim Culver of E. A. White Construction CO., pulling the pins on the above
Simpson Strong-Tie ATS anchor tie-down system units in order to activate their hold down function.
The acceptance criteria for each Simpson Strong-Tie ATS anchor tie-down system unit is as follows: On each unit, there
is a pin that must be pulled in order for the ATS to be functional. After the pin is pulled, the ATS spring must remain intact
and must not show any observable signs of unwinding. During our previous visit, EEI received verbal confirmation of
these criteria by Simpson Strong-Tie representative Jim McPherson. To the best of our knowledge, the work inspected
was in accordance with the above criteria.
The location and type of each Simpson Strong-Tie ATS anchor unit, and whether or not each unit met the acceptance
criteria, was clearly marked on the drawing "2nd Floor Framing Plan, 1-S2.2, Sheet OL, Job No:-130915A, Candlewood
Suites", dated 11/11/2013, provided to us by Jim Culver of E. A. White Construction Company.
Detailed drawings of each type of ATS unit (T1, Ti S, T2, T2S, T3, T3S) were provided to us by Jim Culver of E. A. White
Construction Company (Candlewood Suites Springfield, OR Job No:-130915A, Simpson Strong-Tie). The detailed
drawings state that"The ATS runs in these drawings are based on structural drawings by Ryan Hardie PE, SE.".
On the drawing, "2id Floor Framing Plan, 1-S2.2, Sheet OL, Job No:-130915A, Candlewood Suites", dated 11/11/2013,
the location of each ATS was circled, the type was noted, and a checkmark was placed by the notation if the ATS met the
acceptance criteria. If the ATS did not meet the acceptance criteria, a "?" mark was placed by the notation. Based on our
inspection, both of the replaced ATS units that were inspected met the acceptance criteria. The locations of the (2)
anchor units (at grid lines 9 at M and approximately 9 at J) were noted on drawing "2nd Floor Framing Plan, 1-S2.2, Sheet
OL, Job No:-130915A, Candlewood Suites", dated 11/11/2013, with the type name"T2" and a check mark.

The acceptance criteria for each Simpson Strong-Tie ATS anchor tie-down system unit is as follows: On each unit, there
is a pin that must be pulled in order for the ATS to be functional. After the pin is pulled, the ATS spring must remain intact
and must not show any observable signs of unwinding. EEI received verbal confirmation of these criteria by Simpson
Strong-Tie representative Jim McPherson. To the best of our knowledge, the work inspected was in accordance with RFI
No. 12 and 16, and with the above criteria except as noted below for two ATS units.
The location and type of each Simpson Strong-Tie ATS anchor tie-down unit, and whether or not each unit met the
acceptance criteria, was clearly marked on a drawing (2nd Floor Framing Plan, 1-S2.2, Sheet OL, Job No:-130915A,
Candlewood Suites, dated 11/11/2013) provided to us by Jim Culver of E. A. White Construction Company. The ATS
units are typically located at pop-out shear walls, which are shown as dashed lines on the above referenced drawing (see
photo 1).
Detailed drawings of each type of ATS unit (T1, T1S, T2, T2S, T3, T3S) were provided to us by Jim Culver of E. A. White
Construction Company (Candlewood Suites Springfield, OR Job No:-130915A, Simpson Strong-Tie). The detailed
drawings state that "The ATS runs in these drawings are based on structural drawings by Ryan Hardie PE, SE.". These
details show each typical ATS system, including the floor on which the system terminates. Per verbal confirmation by the
contractor, Jim Culver, each ATS unit, once functional, will be in either tension or compression. Whether the unit is in
tension or compression will change due to the normal shifting and settling of the building structure.
A T3 ATS unit located on gridline 10, between lines A and B, was damaged during the inspection but was replaced and
successfully passed inspection during the site visit on April 11, 2014 (see photo 2). Note that any changes to the design
and location of ATS units are clearly noted on the referenced drawing (2nd Floor Framing Plan, 1-S2.2, Sheet OL, Job
No:-130915A, Candlewood Suites, dated 11/11/2013) and are referenced to either RFI No. 12 or 16.
On the drawing (2nd Floor Framing Plan, 1-S2.2, Sheet OL, Job No:-130915A, Candlewood Suites, dated 11/11/2013),
the location of each ATS was circled, the type was noted, and a checkmark was placed by the notation if the ATS met the
acceptance criteria. If the ATS did not meet the acceptance criteria, a "?" mark was placed by the notation. Based on our
inspection, all ATS units met the acceptance criteria, except for two (2) units located at grid lines 9 at M and
approximately 9 at J, which are marked on the above referenced drawing. These two units at 9 at M and 9 at J were
incorrectly installed and will be replaced and re-inspected on Monday April 14, 2014 per notification of our office by Jim
Culver. The results of that inspection will be presented under separate cover.
