Inspection of: Epoxy Anchors Weather: Clear Continuous Inspection: ❑

Performed continuous inspection during the installation of epoxy anchors as follows:
eight (8) 1 1/4" x 13" stainless steel all-thread anchors into north wall for hold-down
attachment and eight (8) 5/8" x 6" galvanized all-thread anchors into top of wall for
attachment of intake #1 roof structure. Prior to installation, the predrilled holes were
cleaned with a brush and compressed air. Verified minimum embedment depth of 4"
for 5/8" anchors. Measured an embedment of 10" to 10 1/2" for 1 1/4" anchors.
Anchors were secured with Hilti Hit RE 500SD (ICC — ES ESR 2322/ expiration date
09/2014).

Note: plans specify the use of epoxy thru-bolts with a plate and nuts on opposite side
of wall from hold-down. Due to inaccessibility of opposite side of wall (below grade);
Dave Shaw (JHC) stated that approval from EWER was obtained to eliminate thru-bolt
installation and use epoxy anchors.
To the best of our knowledge, the work inspected was in accordance with plans dated
04/2012 and applicable workmanship provisions of the I.B.C., except if noted above.
