As requested by W.W. Construction, Inc., Earth Engineers, Inc. (EEI) representative Ray Aliperti was on-site today to
observe exposed parking lot and driveway pavement subgrade soils within Tax Lot 100 (see attached site plan). Prior to
our arrival the area had been excavated to finished subgrade elevation using a John Deere 160C LC excavator with a 4'
wide smooth bladed bucket. According to W.W. Construction representatives the area was excavated to allow for 12" of
crushed rock and 3" of asphaltic concrete. The exposed soils appeared to be similar fill soils as described during our site
visit on January 27, 2014 (reference our report 13-029-37). Due to the rain making the surface of the subgrade very slick,
the subgrade could not be proofrolled. Therefore, to assist us in determining the stability of the exposed soils we used a
geotechnical soil probe. Based on soil probe penetrations of less than 6", the subgrade was relatively firm and stable at
this time with the exception of the following area: an area measuring approximately 12' by 15' near the southeast corner
(see attached site plan and photos #1 and #2 below). At this location the uper 3' of subgrade contained an abundance of
pvc pipe. The area was overexcavated to remove the pipe and then the excavated material replaced with additional
crushed rock.
To the best of our knowledge, the work observed on this date was in accordance with our previously submitted
geotechnical report entitled " Geotechnical Investigation Report, Proposed Candlewood Suites Hotel, 3005 Franklin
Boulevard, Tax-Lots 100 and 700, Springfield, Oregon, EEI Project No. 13-029-1" dated April 2, 2013; as well as our
report 13-029-37 dated January 27, 2014.

observe exposed parking lot and driveway pavement subgrade soils within Tax Lot 100 (see attached site plan). Prior to
our arrival the area had been excavated to finished subgrade elevation using a John Deere 160C LC excavator with a 4'

crushed rock and 3" of asphaltic concrete. The exposed soils appeared to be similar fill soils as described during our site
visit on January 27, 2014 (reference our report 13-029-37). Due to the rain making the surface of the subgrade very slick,
the subgrade could not be proofrolled. Therefore, to assist us in determining the stability of the exposed soils we used a
geotechnical soil probe. Based on soil probe penetrations of less than 4" (see photo #3 below), the subgrade was
relatively firm and stable at this time.
PM: Returned in the afternoon to observe the remaining exposed subgrade soils for the parking lot and driveway
pavement subgrade soils within Tax Lot 100 (see attached site plan). Based on our soil probe penetrations of less than 4"
to 6" using moderate pressure, the subgrade was relatively firm and stable at this time:
After our subgrade observations in the morning and afternoon, the earthwork contractor immediately covered the areas
with geotextile fabric and imported 3/4"-0 crushed rock from Delta Sand and Gravel. At this time the parking lot and
pavement subgrade areas within Tax Lot 100 are complete. Informed by earthwork contractor that at this time they will
place the 12" of crushed rock and compact it so that the area can be used as temporary parking and storage. At a later
date the rock will be re-graded, compacted, and tested for compactive efforts prior to placement of asphaltic concrete.
To the best of our knowledge, the work observed on this date was in accordance with our previously submitted
geotechnical report entitled " Geotechnical Investigation Report, Proposed Candlewood Suites Hotel, 3005 Franklin
Boulevard, Tax-Lots 100 and 700, Springfield. Oregon, EEI Project No. 13-029-1" dated April 2, 2013; as well as our
report 13-029-37 dated January 27, 2014.

point in the parking lot where the storm water will be drained (see attached site plan and photo #1 below). At this location
the upper 6" of subgrade was saturated and very soft. We recommended removing this material and replacing with
additional crushed rock.
After we observed the subgrade, the earthwork contractor immediately covered the subgrade soils with geotextile fabric
and began placing imported 3/4"-0 crushed rock from Delta Sand and Gravel.

As requested by Jim Culver with E.A. White Construction Company, Earth Engineers, Inc. (EEI) representative Ray
Aliperti was on-site today to perform continuous epoxy anchor inspection on level 1 (first floor). In accordance with RFI
No. 16, a total of thirteen (13) 5/8" diameter T-1 all-thread bolts, four (4) 5/8" diameter T-2 all-thread bolts, and one (1)
7/8" diameter T-3 all-thread bolts were installed into the concrete at the locations in RFI No. 16. Prior to installation, all
holes were cleaned using compressed air and a nylon wire brush. The 2' long all-thread anchors were installed with a
minimum of 10"of embedment, and installed using Simpson Set XP Epoxy with an expiration date of July 2015.
To the best of our knowledge the epoxy anchor bolts inspected above were installed in accordance with engineer
approved RFI No. 16, and the epoxy manufacturer's directions.

As requested by W.W. Construction, Inc., Earth Engineers, Inc. (EEI) representative Ray Aliperti was on-site today to
observe a proofroll on exposed subgrade fill soils within Tax Lot 100 (see attached site plan). Prior to conducting the
proofroll, the area had been excavated to finished subgrade elevation using a John Deere 160C LC excavator with a 4'
wide smooth bladed bucket. The excavation was performed prior to our arrival on site and according to W.W.
Construction representatives; the area was excavated to allow for 12'• of crushed rock and 3" of asphaltic concrete. The
proofroll was conducted using a fully loaded tandem axle dump truck weighing approximately 50,000 pounds. No
deflections (pumping) were observed under the wheel loads and no wheel tracking (rutting) occurred, indicating a firm,
stable subgrade at this time.
After the completion of the proofroll, the earthwork contractor immediately covered the subgrade soils with geotextile
fabric and begun placing imported 3/4••-0 crushed rock from Delta Sand and Gravel (see Photo#1 below).
To the best of our knowledge, the work observed on this date was in accordance with our previously submitted
geotechnical report entitled " Geotechnical Investigation Report, Proposed Candlewood Suites Hotel, 3005 Franklin
Boulevard, Tax-Lots 100 and 700, Springfield, Oregon, EEI Project No. 13-029-1" dated April 2, 2013; as well as our
report 13-029-37 dated January 27, 2014.

perform visual observation of exposed subgrade soils for the parking lot and driveway pavement areas within tax lot 100.
At the time of our site visit, the contractor had excavated the subgrade close to within finished subgrade in the southwest
corner of the lot (see attached site plan). The depth of the excavation ranged from 3' to 4' below existing site grade. The
excavated material, as well as the material exposed close to finished subgrade, visually appeared to be fill material
consisting of a mixture of fine-grained soils (clays and silts) ranging in color from red-brown to black. Within the fill
material were pieces of asphaltic concrete and concrete ranging in size from 4"to 18" as well as pockets of crushed rock.
Based on previously viewed documents and on-site observations, the thickness of the fill prior to any excavation may be
as much as 8' to 10' thick in some areas. At this time the parking lot and driveway pavement areas are being cut to allow
for a minimum of 12 inches of crushed rock underlain with geotextile fabric as recommended in our geotechnical report
entitled: "Geotechnical Investigation Report, Proposed Candlewood Suites Hotel, 3005 Franklin Boulevard, Tax-Lots 100
and 700, Springfield, Oregon, EEI Project No. 13-029-1" dated April 2, 2013. Prior to placing of any geotextile fabric and
crushed rock we recommended proofrolling the exposed subgrade soils to assist us in determining the stability and
firmness of the soils. The proofrolls will be scheduled at a later date as necessary.
During preparation of our geotechnical report we noted in section 1.3 - Purpose and Scope of Services that our
"recommendations for parking lot pavement sections including asphaltic concrete and crushed rock thicknesses for Tax
Lot 700 only. Access to Tax Lot 100 was not available to us at this time because it was being used as a construction
staging area for the I-5 Willamette River Bridge project". We realize it is not economical to remove and replace the 8' to
10' of fill material that underlies the parking lot and driveway pavement areas of tax lot 100. However, the owner and the
project design team need to be aware that there is a risk of future parking lot and driveway pavement distress issues (i.e.
cracking, settling, etc.) due to the underlying fill material. If the owner and the project design team want to greatly reduce
the risk of future parking lot and driveway pavement distress issues within tax lot 100, then the fill material would need to
be removed and replaced with a structural fill as described in our geotechnical report in section 3.2.

Aliperti was on-site today to perform visual inspection during non-shrink grouting under column base plates at the
following locations: eleven (11) 5x5's located at gridlines E @7, E @9, F.2 @13, F.5 @13, H @13, H.2 @13, J @12, J @10,
J @8, G @10, and G @8; and five (5) 6x6's located at gridlines F @10, F.2 @9, 1 @9, I @10, and J @10.8. The non-shrink
grout was placed as required by project details 4/S3.1 (Ext. Int. Wall Columns) and 19/S3.1(Int. Column & Ftg.). Prior to
placing the non-shrink grout the areas under the column base plates were cleaned of water and construction debris using
a vacuum. Dayton Superior 1107 Advantage Grout was used and was mixed in a "dry pack" condition in accordance with
the manufacturer's directions. The material was placed by "hand-packing" it on two sides of each column and using a
hammer and a piece of 2x4 lumber until it began to squeeze out of the other two sides. Then the other two sides were
filled using non-shrink grout and then formed by hand to match the dimensions of the base plate. In accordance with the
project details the thickness of the grout was 1-1/2" or less. A minimum compressive strength was not required per the
project plans and therefore samples for strength tests were not obtained.
To the best of our knowledge the non-shrink grout inspected above was installed in accordance with the City of
Springfield approved project plans and the installation directions of the non-shrink grout manufacturer.

Aliperti was on-site today to perform continuous epoxy anchor inspection. In accordance with RFI No. 12, a total of five
(5) 7/8" diameter all-thread bolts were installed into the concrete footings along gridline Q. The all-thread anchors were
installed with epoxy on this date because the original anchors that were placed prior to concrete placement were placed
- two feet north of their designated locations. As per RFI No. 12, a portion of the concrete stem wall at each location was
removed and 1" diameter holes drilled a minimum of 12" into the concrete footings. Prior to installation, all holes were
cleaned using compressed air and a nylon wire brush. The all-thread bolts, which were approximately 4' long, were
installed.using Simpson Set XP Epoxy with an expiration date of July 2015.
Note: The reinforcing steel bars (horizontal bars within the stem walls) exposed after concrete removal did not appear
damaged.
To the best of our knowledge the epoxy anchor bolts inspected above were installed in accordance with engineer
approved RFI No. 12, and the epoxy manufacturer's directions.

Aliperti was on-site today to perform periodic visual inspection of reinforcing steel for the hotel interior footings at the
following locations: stairwell in the southwest corner along gridline C.7 and 10.5; along gridline G from 6 to 8.8; and along
F from 10 to 12. Reinforcing steel was inspected for grade, size, lap, placement, and clearances where applicable. The
reinforcing steel was secured against displacement using tie wire and supported on plastic chairs to maintain proper
clearances.
Note: The contractor asked us to note that a vapor barrier had been placed over the exposed crushed rock surface
beneath the proposed concrete slab-on-grade being poured today from gridline A to H; electrical conduit was placed for
the proposed hotel exercise room and lobby, and that all ice and snow was removed prior to placement of concrete today.
While on site we also cast one set of concrete placed in the above footing locations as well as the concrete slab-on-grade
from gridline A to H. As per the approved project plans, continuous inspection during the placement of concrete was not
required. One set of four (4" X 8") concrete test cylinders were cast from Knife River's mix no. 2430NEA2M0 (3,000psi,
no air, with midrange reducer) placed in the above locations on this date. The test cylinders were placed in a cooler and
stored in the contactor's office (temperature set at 55° F) for initial storage. Due to the jobsite being closed for the
weekend, they will be picked up on Monday December 16, 2013 and transported to the laboratory for final storage and
compression testing. One test cylinder will be tested at 7-days and the remaining three cylinders tested at 28-days. The
results of those tests along with more detailed concrete information (i.e. time sampled, concrete temperature, etc.) will be
presented in a separate EEI report no. 13-029-32. The sample we obtained had a measured slump of 5".
To the best of our knowledge the reinforcing steel inspected above was in accordance with the City of Springfield
approved project plans.

steel was secured against displacement using tie wire and supported on plastic chairs to maintain proper clearances.
We also cast one set of concrete placed in the above locations as well as for the concrete slab-on-grade being poured
from gridline H to Q. As per the approved project plans, continuous inspection during the placement of concrete was not
required. One set of four (4" X 8") concrete test cylinders were cast from Knife River's mix no. 2430NEA2MO (3,000psi,
no air, with midrange reducer) placed in the above locations on this date. The test cylinders were placed in a cooler and
stored in the contactors office (temperature set at 55° F) for initial storage and will be picked up within 48 hours and
transported to the laboratory for final storage and compression testing. One test cylinder will be tested at 7-days and the
remaining three cylinders tested at 28-days. The results of those tests along with more detailed concrete information (i.e.
time sampled, concrete temperature, etc.) will be presented in a separate report. The sample we obtained had a
measured slump of 5".

under the reinforcing steel bars to maintain proper clearances. The interior of the forms were clean and free of debris and
were sized in accordance with the approved project plans.
We also cast one set of concrete placed in the above locations. As per the approved project plans, continuous inspection
during the placement of concrete was not required. One set of four (4" X 8") concrete test cylinders were cast from Knife
River's mix no. 2430NEA2M0 (3,000psi, no air, with midrange reducer) placed in the above locations on this date. The
test cylinders were placed in a cooler and stored in the contactor's office (temperature set at 55° F) for initial storage and

As requested by Jim Culver with E.A. White Construction Company, Earth Engineers, Inc. (EEI) representative Ray
Aliperti was on-site today to perform periodic visual inspection of reinforcing steel, anchor bolts, and concrete forms for
the hotel interior footings except at the following locations: stairwell in the southwest corner along gridline C.7 and 10.5,
stairwell in the northeast corner'along gridline P and 2.2, along gridline G from 6 to 8.8, along J from 4 to 8, along F from
10 to 12, and along I from 10.7 to 12. At these locations the footings will be poured monolithically during the placement of
the concrete slab-on-grade. Reinforcing steel and anchor bolts were inspected for grade, size, lap (reinforcing steel only),
placement, and clearances where applicable. The reinforcing steel and anchor bolts were secured against displacement
using tie wire and wooden templates. Plastic chairs were used under the reinforcing steel bars to maintain proper
clearances. The interior of the forms were clean and free of debris and were sized in accordance with the approved
project plans.
We also cast one set of concrete placed in the above locations. As per the approved project plans, continuous inspection
during the placement of concrete was not required. One set of four (4" X 8") concrete test cylinders were cast from Knife
River's mix no. 2430NEA2M0 (3,000psi, no air, with midrange reducer) placed in the interior footings on this date. The
test cylinders were left on-site in a cooler for initial storage and will be picked up within 48 hours and transported to the
laboratory for final storage and compression testing. One test cylinder will be tested at 7-days and the remaining three
cylinders tested at 28-days. The results of those tests along with more detailed concrete information (i.e. time sampled,
concrete temperature, etc.) will be presented in a separate report. The sample we obtained from the first concrete truck
had a measured slump of 5".
To the best of our knowledge the work inspected above was in accordance with the City of Springfield approved project
plans.
