Inspection of: Reinforced Concrete Weather: Overcast Continuous Inspection: ❑
Contractor: CCC BP No.: 811-SPR2014-00095 Periodic Inspection:
Performed inspection on the reinforcing steel for eight (8) column footings. Reinforcing steel was
checked for size, grade, quantities, locations, ties, clearances and supports. Anchor bolls were
checked for size, quantity and embedment, and were secured in-place with templates.
Observed the placement of twenty (20) cubic yards of Eugene Sand & Gravel's 3,000-ps1 concrete
(mix 4313068). Concrete was placed directly from truck and was mechanically consolidated. Cast
one (1) set of five (5)4"x 8"compressive-strength cylinders. Test results are listed below:
• Slump: 5 1/4"; concrete temperature: 55° F; ambient temperature: 36° F.
Notified Shawn (CCC)of on-site results.
To the best of our knowledge, the work inspected was in accordance with plans dated 10-15-12 and
applicable workmanship provisions of the IBC, except if noted above.

Met on-site with Scott Rogge (WBC) to observe exposed subgrade soil conditions
in the proposed building 5 slab area and to observe conditions around the old septic
tank that was encountered near the southwest building corner.
In order to ascertain stability, a proofroll was conducted using a loaded double axle
dump truck weighing approximately 50000 lbs. The materials appear relatively
stable under load with the following exceptions: soft deflecting areas were identified
along the existing building; around the septic tank; near the proposed northwest
building corner (possible previously excavated test pit for geotechnical report) and
at two (2) locations along the south side building line. Recommended unstable
areas be over-excavated to firm, stable material prior to placing crushed rock base.
Also, discussed options for septic tank area. Mr. Rogge is to contact owner
regarding complete removal of tank and surrounding soft soil.
Prior to leaving site, suggested that constructing temporary thickened truck travel
lanes be considered to prevent disturbance to subgrade soils during excavation and
footing construction operations.
Mr. Rogge was asked to notify our office when additional services are needed.
