Performed continuous inspection during the installation of two (2) 1/2" x 6 1/2" and one
(1) 5/8" x 16" all-thread anchors into slab and footing for attachment of plumbing wall
hold-downs. Prior to installation, the predrilled holes were cleaned with a brush and
compressed air. Verified minimum embedment depth of 8" for 5/8" anchor and 2 3/4"
for 1/2" anchors. Anchors were secured with Hilti Hit RE 5005D epoxy(ICC — ES ESR
2322/expiration 09/2014).

To the best of our knowledge, the work inspected was in accordance with RFI 7 and
applicable workmanship provisions of the I.B.C., except if noted above.
