Fenstermacher ID No.: 5258882-49 ICC Inspection of: Epoxy Anchors Weather: Inside Continuous Inspection: ❑ Contractor: Chambers Construction BP No.: 811-SPR2013-02200 Periodic Inspection: fJ • Performed continuous inspection during the installation of two (2) 1/2" x 6 1/2" and one (1) 5/8" x 16" all-thread anchors into slab and footing for attachment of plumbing wall hold-downs. Prior to installation, the predrilled holes were cleaned with a brush and compressed air. Verified minimum embedment depth of 8" for 5/8" anchor and 2 3/4" for 1/2" anchors. Anchors were secured with Hilti Hit RE 500SD epoxy(ICC— ES ESR 2322/expiration 09/2014). • To the best of our knowledge, the work inspected was in accordance with RFI 7 and applicable workmanship provisions of the I.B.C., except if noted above. Delatte ID No.: 1080867-88 ICC Inspection of: Epoxy Dowels Weather: Inside Continuous Inspection: ❑ Contractor: McKenzie Commercial BP No.: 811SPR-2013-01766 Periodic Inspection: E1 • Observed the installation of approximately one hundred forty-eight (148) #4 x 24" horizontal dowels and thirteen (13) #5 x 30" horizontal dowels into existing slab on • grade/footing for new slab on grade/footing connection at the new restroom/ locker room addition. Prior to installation, the predrilled holes were cleaned with a brush and compressed air. Verified minimum embedment depth of 3" or 4". Dowels were secured with Simpson Set XP epoxy adhesive (expiration date 05/2015).' To the best of our knowledge, the work inspected was in accordance with plans dated 07-19-13 and applicable workmanship provisions of the I.B.C., except if noted above. Reviewed By: Michael L.
