Inspection of: Epoxy Dowels/ Weather: Overcast Continuous Inspection: ❑
Reinforcing Steel
Contractor: John Hyland Construction BP No.: 513-1789 Periodic Inspection: Q
Observed the installation of#8 x 36" dowels into existing concrete walls for new shear
wall infill as follows: fourteen (14) vertical dowels along the bottom; sixteen (16) vertical
dowels along the top and four (4) horizontal dowels on each side of new infill. Prior to
installation, the predrilled holes were cleaned with a brush and compressed air.
Verified minimum embedment depth of 18". Dowels were secured with Hilti Hit Hy
200A/200R epoxy adhesive (ICC—ESR 3187/expiration date 06/2014).
Also, performed preliminary reinforcing steel inspection for new shear wall at man door.
Reinforcing steel was checked for grade, size, lap, placement, quantities, ties and
clearance.
To the best of our knowledge, the work inspected was in accordance with plans dated
05-28-13 and applicable workmanship provisions of the I.B.C., except if noted above.
