Performed continuous inspection during the installation of ten (10) 5/8" x 7" all-thread
anchors into concrete wall at roofline for the attachment of wall opening support. Prior
to installation, the predrilled holes were cleaned with a brush and compressed air.
Verified minimum embedment depth of 5". Anchors were secured with Hilti Hit RE
500SD (ICC— ES ESR 2322/expiration date 02/2014).
To the best of our knowledge, the work inspected was in accordance with permit
submittal plans dated 09-26-13 and applicable workmanship provisions of the I.B.C.,
except if noted above.

Inspection of: Reinforced Concrete Weather: Inside Continuous Inspection: ❑

Performed inspection on the reinforcing steel in two (2) post footings in rooms 238 &
240. Rebar was checked for grade, size, lap, spacing, quantity and clearances, where
applicable. Rebar was secured with tie wire and was supported above grade with dobie
blocks.
Observed the placement of 3.0 yards of Eugene Sand and Gravel 3500 psi concrete.
Concrete was placed with wheelbarrows and was manually consolidated. Cast one (1)
set of four (4) 4" x 8" compressive strength cylinders. Slump — 3 3/4"; concrete
temperature—64F; air temperature—55F.
Tim Jacobs (CCC) was advised of the on-site test results and was asked to notify our
office when additional services are needed.
To the best of our knowledge, the work inspected was in accordance with approved
plans, RFI 10 (pending) and applicable workmanship provisions of the I.B.C., except if
noted above.
