BP No.: 811-SPR2012-02519 Periodic Inspection: Performed nailing inspection on the re-installed interior face of the W3 & W6 131 floor and 2nd floor shear walls on line 5.1, between lines F and G; on 2, between C and E and on F, between 3 and 5. The 15/32" APA rated plywood was secured to the framing members with 0.148 x 2 1/2" nails. Verified nail spacing at panel edges and at intermediate studs conforms to requirements specified in shear wall schedule. Note: replacement CS 16 straps are not yet installed on 2nd floor wall on line 2. Mel Berkshire (CCC) was advised and was asked to notify our office when additional services are needed. To the best of our knowledge, the work inspected was in accordance with plans dated 12-19-12, RFP's 15, 18 and 19 and applicable workmanship provisions of the I.B.C., except if noted above. Reinforcing steel was checked for grade, size, lap, placement, quantities and clearance. Reinforcing steel was secured and supported above grade. Observed the placement of 35.0 yards of Eugene Sand and Gravel 5000 psi concrete (mix 390531). Concrete was placed directly from truck and was mechanically consolidated. Slump–4"; air temperature–60F; concrete temperature–80F. To the best of our knowledge, the work inspected was in accordance with emails dated 08-13 & 15, 2013 and applicable workmanship provisions of the I.B.C., except if noted above. Test locations were prepared and the kits were installed per manufacturer's instructions. The test kits were left in place for approximately 69.5 hours and were then collected on August 26, 2013.
