LOCATED AT: 4022 Camellia
APPLICANT: Bob Smalley
Plans submitted for the proposed construction at the above project addreis
have been reviewed for conformance with the City of Springfield building
safety codes. Investigation of existing construction for the structure
which was built without required permits and inspections revealed
conditions requiring modification or repair. The following list of items
require correction, clarification or additional work before they can be
approved: '
~Install' a GFI receptacle in the exterior outlet.
~.The junction box in the attic shall be made accessible and
provided with an approved cover. secure all electrical cables within
12" of the box.
~Install a cover plate on the outlet for the washing machine.
~Install a "blank" switch in the 3~gang switch plate at the patio
d7or.
5 Install a ceiling outlet box for the fan.
~~~ Disconnect the hot tub wiring or rewire same with a properly
~ounded system using approved electrical conduit (gray plastic or
meta-I) . ,. ~1 'i-U~
~ Install additional insulation in the accessible atti area over
the f,amily room to compensate for the 2x4 wall c~,u ion. The
requited additional insulation value requir~d i~'; (note: the
existing attic insulation was measured at a" to 10" of loose fiber.
The ~ew insulation should be placed on top of the existing material)
0he water piping supplying the exterior lawn faucet shall be
adequately supported using approved hangers.
9/provide requested information regarding the type, size and
qua~tity of screws used to connect the existing strong-back beams to
the glu-Iam beam in the attic~

Note: It is noted that a new 4x12 beam was installed and inspected
at the ridge of the vaulted ceiling to support the roof loads in lieu
of required collar ties.

OTHER DATA SHALL NOT BE CONSTRUED TO BE A PERMIT FOR, OR AN APPROVAL OF,
ANY VIOLATION OF ANY OF THE BUILDING SAFETY CODES OR OF ANY OTHER
ORDINANCE OF THE CITY OF SPRINGFIELD.
ANY CHANGES OR ALTERATIONS made after the, date of this review must be
approved by the Building Official.
