information hereon is true and correct, and I further certify that any and all work performed shall be done in accordance with the
Ordinances of the City of Springfield and the Laws of the State or Oregon pertaining to the work described herein, and that NO
OCCUPANCY will be made of any structure without permission of the Community Services Division, Building Safety. I further
certify that only contractors and employees who are in compliance with ORS 701.005 will be used on this project. I further agree
to ensure that all required inspections are requested at the proper time,that each address is readable from the street,that the
permit card is located at the front of the property, and the approved set of plans will remain on the site at all times during
construction.

Option A:$16;495.00(Main building only)
1.) Remove and dispose of exiting roof
2.) Install poly separation barrier.
3.)Install a new 60 mil. TPO membrane roof
4.) Install all roofflashings.
5.)Install new coping metal.
6.) Clean ground and gutter of debris from roofing project.
Option B:$13,325.00 (Main building only, recover)
1.) Prep roof for recover, then same as Option A

NOTE:During roof removal, debris may sift into attic. Please cover any valuables.
McKenzie Roofing Inc. guarantees their workmanship on the project for three years
upon completion.All material is guaranteed to be as specified, and the above work
to be performed in accordance with the drawings and specifications submitted for
above work and completed in a substantial workman like manner for the sum of(SEE
ABOVE) with payment due in full upon completion of project
Any alteration or deviation from the above specifications involving extra
cost,will be executed only upon written orders, and will become an extra charge
over and above the estimate.
All agreements contingent upon accidents, delays, or material price increases beyond our control. Owner
to obtain permits, carry fire, tornado and other necessary insurance upon above work.
