BENTONVILLE, AR 72716-5570 NUGENT, DALE RUSSELL, DAVID S, ERIC �EVENSON,ON,KA KATYRYN Project: WM 4178-00 SPRINGFIELD OR TO JERKOVICH, ROBERT SPRINGFIELD, OR CASTILE,WILSON HU Approved Signatory: Deborah Priest(Special Project Manager) Dale of Issue: 6114/2013 Date: 6/10/2013 Technician: Dale Russell Made of Steel is on site setting the 4" x8" x 3/8" bent plate w/6 headed nelson studs over the dock door opening at gridlines 2.6 from A.2 to A.3, also placed were (2)4" x 8" x 3/8" bent plate (1) per side of the above mentioned opening , using (7) 3/8" x 7" all thread rods per side, 14 total were placed, the holes were 7/8" dia, for the nelson studs and 7/16"for the 3/8" rod. Both were drilled to a minimum measured depth of 6", bottle brushed and blown clean with the use of compressed air prior to the placement of epoxy. Made of Steel also placed (2) 10'-8"x 101/2" "C" channel headers over the truck dock door opening at gridlines 2.6 from A.2 to A.3 and A.4 to A.5, (11) per side at 1'-4" o.c. with /2 at each end per detail 1 of sheet S5, using 3/4"x 9" all thread rods, 22 total were placed. The 7/8" holes were drilled to a minimum measured depth of 6" then bottle brushed and blown clean with the use of compressed air prior to the placement of the epoxy. The approved epoxy used was Hilti HIT HY-70, ICC ESR 3342, Expiration date 2-14. Crestview Construction is preparing the (3) F4, 3'-0"x 3'-0" x 1,-0"footings for the Bale and Pallet storage area on the Northwest side of the existing building, the footing subgrade area were probed with a standard 5/8" "Geo" probe. Area was found to be firm and unyielding with penetrations of less than 1/2" in any location observed. Dynasty Concrete Co. is onsite setting the forms and placing the reinforcing steel for the above mentioned (3) F4 footings for the Bale and Pallet Storage Area, The reinforcing steel placed consisted of a mate with (4)#4's each way. Concrete is scheduled for 6-11-13. Al Straightline is on site preparing the store front area for the placement of bollards. Al is using a Bobcat with an attached 24" auger, approxmately drilling a 4'-4" deep foundation. The prepartion of each foundation hole bottom is hand tamped prior to the placement of approximately 3" minimum thick pad of 5000 psi, high-strength "SAKCRETE" site mixed concrete.
