Performed continuous inspection during the installation of Hilti KB-TZ wedge anchors
into the bottom of the 6` floor slab for connection of the patient lift brackets and
supports as follows: room 5F37—fourteen (14) 1/2" x 5 1/2"; room 5C13—nine (9) 1/2"
x 5 1/2" and two (2) 1/2" x 7". Prior to installation, the predrilled holes were cleaned
with a vacuum. Verified minimum effective embedment depth of 3 1/4". Installation
torque of 40 ft/lbs. was verified with a calibrated torque wrench.
Kirk Haskell (PeaceHealth) was asked to notify our office when additional services are
needed.
To the best of our knowledge, the work inspected was in accordance with Alpha
Modalities drawing S1.1 dated 06-05-13 and applicable workmanship provisions of the
I.B.C., except if noted above.
