Inspection of: Epoxy Anchors Weather: Overcast Continuous Inspection: ❑
Contractor: Chambers Construction Co. BP No.: 811-SPR2012-02519 Periodic Inspection: IEl
Observed the installation of four (4) 5/8" x 12" and one (1) 5/8" x 8" all-thread anchors
into slab on grade/footing for hold-down connection near lines 2.4C, 3C, 5.5C, 2.6D
(12") and 5E (8"). Prior to installation, the predrilled holes were cleaned with a brush
and compressed air. Verified minimum embedment depth of 6". Anchors were
secured with Hilti Hit Hy 150 Max SD epoxy adhesive (ICC— ESR 3013/expiration date
11/2013). Contractor stated nuts will be installed after epoxy has set.
To the best of our knowledge, the work inspected was in accordance with plans dated
12-19-12 and applicable workmanship provisions of the I.B.C., except if noted above.
