Inspection of: Epoxy Anchors Weather: Overcast Continuous Inspection: ❑
Contractor: Chambers Construction Co. BP No.: 811-SPR201 2-02 51 9 Periodic Inspection: El
Observed the installation of sixteen (16) 3/4" x 10" all-thread anchors into slab on
grade/footing for column to slab connection within lines D.5 to F, from 2.5 to 6.
Contractor installed two (2) anchors at four(4) locations and four(4)anchors at two (2)
locations. Prior to installation, the predrilled holes were cleaned with a brush and
compressed air. Verified minimum embedment depth of 7". Anchors were secured
with Hilti Hit Hy 150 Max SD epoxy adhesive (ICC — ESR 3013/ expiration date
11/2013). Contractor stated columns and nuts will be installed after epoxy has set.
To the best of our knowledge, the work inspected was in accordance with plans dated
12-19-12 and applicable workmanship provisions of the I.B.C., except if noted above.
