Performed continuous inspection during the installation of fifty-two (52) 3/4" x 24"
vertical and horizontal all-thread rods into the existing east wall of Intake Building 1 at
three (3) infill locations. Prior to installation, the predrilled holes were cleaned with a
brush and compressed air. Verified minimum embedment depth of 6". Rods were
secured with Hilti Hit RE 500SD epoxy(ICC— ESR 2322/expiration date 01/2014).
Dave Shaw(JHC)was asked to notify our office when additional services are needed.
To the best of our knowledge, the work inspected was in accordance with addendum 2
drawings dated 05-10-12 and applicable workmanship provisions of the I.B.C., except if
noted above.
