Inspected for required clear space of 3"for the bottom and sides.

Remarks:
On site at contractors request to perform periodic visual inspection of the(2)truck dock leveler pits at grid lines A.1 to A.4 form 2.4 to 2.6.
Dynasty Concrete Co. has placed the#3'at 12"on center each way for the bottom matt of each dock leveler pit and has also placed the
#3 `Z"bars (2'-0"x 1'-8" x 1'-8") at 12"on center around the perimeters of both pits. Dynasty has also drilled into the existing slab on
grade 6"for the a/n"x 1'-0"smooth dowels at 24"on center to be placed with grease into the existing slab end as per detail 1/S2.1.
Dynasty has also drilled the 6"deep holes for the#3 x 4'-0" (2'-0"x 2'-0") hooked dowels to be placed along grid line 2.6 as per details
12/S2. These holes will be epoxied in at a later time. Some preliminary form work has been completed for the sides and front. This work
in ongoing.
To the best of PSI's knowledge,the work inspected to the approved construction documents are:
