(DRC2004-00004) Decision to be
issued, Final Site Plan submittal and
Development Agreement to be
signed. An LDAP will also be
required. Denise Walters is planner.
Talked with Terry Wellner and
faxed him correction list.
Received amended site plan and
foundation plan for residence.
Still need bid amount for paving,
talked with Terry on 3-29. Revised
structural and final site plans where
submitted. Site plan approval
subject Pearson's approval.
Received fax from Terry Wellner
with truss for office/residence, and
agreement with Eric Gambee.
Called Terry Wellner and asked for
a breakout by the Contractor
between buildings and site work.
Also asked for the truss engineering
"by others" that is missing.
4/5/2004 Called Terry and reminded
him that I still need these requested
items.
Talked with Rich Silkett project
engr. and faxed him a copy of
corrections and concerns as of 2-25

information hereon is true and correct, and I further certify that any and all work performed shall be done in accordance with
the Ordinances of the City of Springfield and the Laws of the State of Oregon pertaining to the work described herein, and
that NO OCCUPANCY will be made ofany structure without permission of the Community Services Division, Building Safety.
I further certify that only contractors and employees who are in compliance with ORS 701.005 will be used on this project.
I further agree to ensure that all required inspections are requested at the proper time, that each address is readable from the
street, that t rmit card is located at the front of the property, and the approved set of plans will remain on the site at all
