Wastes: No Waste Date Received: OCT = 1 2012 Original Submittal— Revised:Sept 26,2012(RZ) I 0 0 Section V: Record Keeping (Inspection and Maintenance) Description of our Inspection Program: Our On-Site Manager, Frank Warm, is in charge of performing monthly visual inspections of the secondary containment devices and insuring repairs are promptly made if tubs or containment pallets-become cracked or any cracks'are observed in the floor-system within the recessed containment area (see section V-A below for specific instructions). Note the containment pallets will have electronic moisture sensors that will alarm should the primary container fait.The batteries in these devices will be replaced on an yearly basis, starting with the anniversary of the opening of the new facility. The-monthly inspection shall_be recorded on the attached inspection form and put in the HMMP Binder to be kept within the on-site office for no less than seven years. This information will'be available for inspection by a representative of.the City of Springfield or SUB-at any time during business hours, prior notice is requested to insure Frank Wann or a delegated representative will be on site to assist and answer questions. Section V-A ( Repair or replacement Procedures) If a plastic containment tray or a containment pallet becomes cracked or broken it shall be replaced with like kind immediately. The recessed concrete floor Secondary Containment Area is to be made non-permeable by installing "Dayton Superior" 'Day-Chem Sure Hard' in accordance with the manufacturers installation instructions. If a crack is observed in the concrete containment floor it shall be repaired using the following method within 48 hours of the initial observation. Clean the crack with a wire brush and sweep or vacuum the area to remove all foreign debris Install "Dayton Superior" 'Sure Inject jS6' using the applicator to inject into crack and in accordance with manufacturers specifications. When material fs.cured, cut flush with floor to minimize opportunity for pulling material out of crack. Perform this as often as is necessary to insure the floors integrity for containing any breaches or spills. Section VI: Spiil Prevention All listed materials will be stored in original shipping containers or.packaging on securely attached storage racking system within the secondary Containment Area until used. All listed materials will be stored on the aisle shown on provided plan. The only non-compatible materials are the Phosphoric Acid and Quantum-H (Humic), these materials will be kept at least 20 Lf. from each other. The Phosphoric Acid will be kept on containment pallets with electronic moisture alarms within each chamber. All the. listed materials will be kept on the floor or at eye-level so that product name and NFPA hazard diagram are visible from the aisle. Transfer of the listed materials that are liquid shall be by portable pump connected directly to mixing tanks. The pump will be kept in a plastic tray when not in-use. Dry materials will be transferred from manufacturers packaging directly into mixing tanks.The packaging shall be discarded in accordance with manufacturers guidelines or recycled to supplier. In the event there is a spill of any listed material during unloading at the Loading Dock the sump-pump, which is connected directly to the sanitary sewer. shall be immediately turned off, 911 contacted, and spill cleaned up in accordance with Hazardous Materials Response Plan. The On-Site Manager is responsible for maintaining this documentation and keeping it in an orderly fashion. The HMMP shall be kept in its entirety in a binder located in the on-site office. Section X: Notes e Wellhead protection signs are installed on-site to inform staff and visitors of the importance of reporting and cleaning up spills. These signs are located front of Building'A' facing the parking area and on Building'C' adjacent to the N.W. Person door, as shown on the attached plans. Damaged or missing signs should be replaced. • The Loading Dock area is protected by a trench drain connected to a sump with pump to the sanitary sewer system. There is to be an "Emergency Shut-Off' at the Dock adjacent to the Overhead Door. Provide a sign above "Emergency Shut-Off' button that says, "In the event of any material spill within the Dock Area or Truck Well press button to shut off sump pump". This sign shall be posted in English and Spanish.
